History
The Chronic Care Collaborative (CCC) first began meeting in 2009. Realizing that most voluntary health organizations were advocating for similar healthcare goals, the idea for the CCC was born. The CCC started with 24 organizations and 7 priorities, with the goal to represent and jointly advocate for the 1 in 4 Coloradans living with a chronic disease. Over the years the CCC has successfully advocated for policies, regulations, and legislation which directly impact the lives of Coloradans living with a chronic disease. From advocating for expanded access to affordable healthcare services to offering respite care for caregivers the CCC is dedicated to increasing life enhancing opportunities for Coloradans living with chronic disease, their families, and friends
